Minor Consent Form
Print, fill out and have this form notarized if you would like to give consent to your child for a tattoo or piercing. Parent and Minor must BOTH have a valid, state issued ID. Please Note: Minors must be the age of 16 in order to receive our services. Also, Minors receiving repeat services, will need a notarized form and a parent present for each visit. Unfortunately, Chicago tattoo Co does not have the power to make any exceptions. Chicago Tattoo Co supports and exceeds these regulations keeping Orlando’s safety as our priority.
